Cardiac rehabilitation services across six areas of Pomerania, for licensed medical providers with a cardiac team
A medical provider will deliver a comprehensive cardiac rehabilitation programme in six parts, each covering a defined group of districts in the Pomeranian region. The programme includes diagnostics, physical training, risk-factor modification, education, psychological support and cardiology consultations, and the provider also recruits patients into the project. The contract is financed under project agreement FEPM.05.05-IZ.00-0002/25.
Who can win it
- Must be a licensed medical entity with a cardiology, internal medicine or rehabilitation clinic, ward or outpatient service
- Must have or commit to a cardiac rehabilitation team: cardiologist or rehabilitation specialist as leader, physiotherapist, psychologist, nurse or medical technician, dietitian, patient carer
- Must have or commit to the required rooms and equipment: kinesiotherapy room, ECG/Holter/ECHO, analytical laboratory, defibrillator, CPR manikin, education and presentation rooms
